What is the Cardiology ST4 interview, and how do you practise for it?

Cardiology ST4 national recruitment interview, run by Physician Higher Specialty Training: two 22-minute online stations, each with its own pair of interviewers. Station 1 asks three questions: suitability and commitment, medical registrar suitability opened by a one-minute presentation, and an ethical, professionalism and governance question given verbally. Station 2 is a written clinical scenario, then a second clinical scenario given verbally, with communication marked across the station. Here one examiner asks the questions. MedMock lets you practise it out loud against an AI interview panel, with examiner-style feedback on any station you finish.

Cardiology ST4, 2025 recruitment round (Round 2). NHS England competition ratios

  • The lead recruiter for cardiology is Wessex.
  • Cardiology is a Group 1 specialty: applicants need to have completed the internal medicine training (IMT) stage 1 programme or equivalent, and all programmes dual specialise with general internal medicine.
  • The interview has two stations of 22 minutes each, with five questions plus a communication mark, giving six scored areas and lasting approximately 50-55 minutes.
  • Interviewees give a one-minute presentation summarising their experience managing the acute unselected medical take as the medical registrar.
  • The specialty does not assess commitment to specialty at shortlisting, scoring applications purely on the self-assessment framework; commitment is assessed at the interview.
  • To be appointable at interview, none of the twelve interview scores can be 1/5, no more than two can be 2/5, and the raw interview score must be 36 or above.

Cardiology ST4 interview practice, on demand

MedMock gives you an AI interviewer who challenges your answers, available 24/7. Pick a station, practise your answers, and request feedback after the session. One automated practice assessor grades your performance; the score is a MedMock practice assessment, not an official PHST panel result.

New versioned Station 1 and Station 2 Cardiology ST4 scorecards use the published 2026 PHST areas and weights: Station 1 uses Suitability and commitment (1.6), Medical registrar suitability (1.2), and Professionalism and governance (1.6); Station 2 uses Clinical scenario 1 (1.6), Clinical scenario 2 (1.2), and Communication (0.8). Each area receives a practice grade from 1 to 5 from one automated assessor, not an official result from two independent human assessors or an application score. When both saved station scorecards use this version, Full Mock pools their marks using the published 22:18 station maxima, omitting unassessed areas. Earlier or mixed-version sessions keep their saved scoring; their Full Mock average is not recalculated to the new weighted scheme. None of these scores includes the separate application score.

How it works

1

Choose a station

Choose the kind of question or scenario you want to rehearse — ethics, motivation, a role-play, and more.

2

Have the conversation

Speak naturally, out loud. The AI interviewer listens and asks the demanding follow-ups, just as a real interviewer would.

3

Get instant feedback

The moment you finish, you get specific, examiner-style feedback and a score — what you did well, what cost you marks, and precisely what to fix before the next attempt.

Practise every scenario

MedMock covers the stations you will face in the Cardiology ST4 interview. Rehearse each one until it feels routine.

1

Station 1 — Suitability, Registrar Suitability and Ethics

Question 1 (about 10 minutes): your suitability for and commitment to cardiology, expanding on your application form. Question 2 (about 7 minutes): medical registrar suitability, opened by your one-minute presentation on managing the acute unselected take. Question 3 (about 5 minutes): an ethical, professionalism and governance situation given verbally, underpinned by GMC Good Medical Practice.

2

Station 2 — Two Clinical Scenarios

Question 1 (about 13 minutes): a written clinical scenario, given a short while to review, then questions on it. Question 2 (about 9 minutes): a second clinical scenario given verbally with no advance notice. Communication is marked across the whole station.

Sample Cardiology (ST4) interview questions

Two questions from the topics that come up most in MedMock's Cardiology (ST4) bank.

Station 2 — Two Clinical Scenarios

“Here's a cross-team conflict. You believe a frail 85-year-old with severe symptomatic aortic stenosis should be discussed for intervention, but the surgical team disagrees. Talk me through how you would handle it.”

Station 1 — Suitability, Registrar Suitability and Ethics

“A patient due for a permanent pacemaker seems unclear about the risks when the registrar checks consent shortly before the procedure, despite having signed a form completed by a junior colleague. What should the registrar do?”

Example scenarios

A sample of the Cardiology ST4 scenarios you can practise, each as a spoken conversation.

Station 1 — Suitability, Registrar Suitability and Ethics

“Tell us why cardiology is the right specialty for you; then give us your one-minute summary of managing the acute unselected take as the medical registrar.”

Station 2 — Two Clinical Scenarios

“A 60-year-old woman with recurrent unexplained syncope is found to have complete heart block. Talk us through your management, and how you would explain the plan and the driving advice to her.”
